Due to the COVID-19 restrictions, we are not scheduling in-person appointments at our office at this time.  All applications will be taken over the phone.  One of our intake staff will give you a call to complete your application on the day of your appointment.

List of Docs to Provide

Failure to provide all required documentation will delay your assistance. We require below documents:

  • Driver's license or picture ID
  • Social Security card(s) or official documentation with social security numbers (or Permanent Resident/Green Card) for everyone living in the household
  • Most recent light and gas bill (MUST be a bill dated within the past 30 days)
  • Proof of income for the last 30 days for everyone who is 18 or older living in the home. If someone 18 or older has zero income, they MUST come to the appointment.
  • NEW REQUIREMENT - If your home is heated with natural gas, kerosene, propane or wood, then you MUST have both your heating bill and your electric bill
